Location

During our discussion, we’ll determine if your care needs are a good fit and if we’re able to provide the level of care required. If so, we’ll share available locations and help you schedule a tour. During the tour, you’ll have the opportunity to meet our staff and other residents. If you decide to move forward, we will complete an evaluation of your loved one to ensure we can meet their specific needs.

Admission Process

We then coordinate with the medical team involved. Whether your loved one is coming from a rehab, hospital, or another facility;  we gather the necessary information so we can provide appropriate care in our setting. If your loved one is coming from home, we will obtain all relevant medical records and information from their primary care provider to ensure a safe and seamless continuation of care.
